What a family pays in 2026
A typical family of four (two adults, two children) pays CHF 1'175.60 a month for basic insurance, about CHF 14'107 a year, roughly CHF 600 more than in 2025. Averages: adults CHF 465.30, young adults CHF 326.30, children CHF 122.50 a month.
Family discounts
Some insurers give 10 to 15% off the children's premium from the 2nd child (e.g. Assura, Sanitas) or the 3rd child (e.g. Agrisano, Concordia, Helsana, KPT, ÖKK, SWICA, Visana). Always compare the final price, a discount can still be dearer than separate insurers.
Cheapest insurers for children
The cheapest insurer for adults is rarely the cheapest for children, so compare children's premiums separately. Agrisano is cheapest in 28 of 42 premium regions, Visana leads in 7, and SWICA is consistently top three in urban areas.
The deductible strategy
For healthy adults, choose the CHF 2'500 deductible and an alternative model (HMO or Telmed), up to CHF 3'000 saved per adult a year. For children, choose the CHF 0 deductible; their maximum out-of-pocket is CHF 350 a year versus CHF 700 for adults.
A worked example
A family of four on the standard model with a CHF 300 deductible pays about CHF 15'600 a year. The same family on HMO with a CHF 2'500 deductible pays about CHF 9'960, a saving of CHF 5'640 a year.
Pregnancy and maternity
Basic insurance covers seven check-ups, two ultrasounds, delivery in hospital, a birth centre or at home, and midwife aftercare. From the 13th week of pregnancy until 8 weeks after birth, the deductible and excess are waived entirely.
Prenatal sign-up
Register your baby between the 5th and 8th month of pregnancy so it is insured from the first minute, including supplementary insurance with no health assessment. Basic insurance must accept every child, but supplementary insurance does not, so prenatal sign-up removes the rejection risk.
Supplementary insurance for children
Dental cover is strongly recommended, basic insurance pays nothing for dental care and orthodontics can cost CHF 5'000 to 15'000 (from about CHF 8 to 15 a month for children). Take it out early and never cancel it, even when switching insurer.
Young adults (19-25)
At 19 the premium jumps from CHF 122.50 to CHF 326.30 a month (+166%); at 26 it rises again to CHF 465.30 (+43%). The change happens automatically on 1 January, so compare insurers in good time and do not cancel supplementary plans, health checks get stricter.
